Insurance 

Health insurance is a contract between you and an insurance company to help you pay medical bills. Such contracts do not involve a physician who sees you as a patient. If you choose to see a physician, it is your responsibility, and yours alone, to pay the bill. Each insurance company has its own set of rules; some covering fixed amounts for certain procedures and others fixed percentages. It is your responsibility to pay any deductible, co-insurance or other balances not paid by your insurance company. If you have a disagreement with your insurance company, you must contact the company to resolve it. The existence of such a disagreement does not alter your direct responsibility for a medical bill. We try to simplify the preparation of insurance claims, thereby holding down the costs that are unrelated to the delivery of good medical care. The receipt form we give you at the end of each visit contains all information needed from the doctor for your insurance company to process the claim. Patients enrolled in managed care MUST present the appropriate referral from their primary care physician in order to be seen. This is a requirement of your insurance company in order to qualify for benefits. Patients arriving without their referral will have the option to reschedule or pay out of pocket for their visit
